The nursing team skillfully identifies each resident’s needs and develops personalised care. From the time of a resident’s arrival and throughout a resident’s stay, the focus of the nursing team is to ensure that each resident has the most appropriate personalised care, provided in a kind and compassionate way.
Each resident has a nominated nurse who undertakes the assessments of the resident and develops the care plans together with the staff team in liaison with families. As time goes on, the nursing team will adapt and shape the care delivered. These adaptations take account of any frailties that a resident may have, any physical illness and changes in a resident’s dementia and behaviour.

At Wardington House, Registered Nurses have a central role in the clinical care and leadership of the home.
Communication with families and health care professionals
This is a key aspect of the nursing care at Wardington House. The nurses liaise with families from the very start and will keep in touch with families during a resident’s time at the home, especially if there are any changes. The nursing team will also liaise with the resident’s GP, physiotherapists, dietitians, and social workers. The nurses coordinate care for residents returning from hospital or attending outpatient appointments.
Clinical care
At Wardington House, the registered nurses are the front-line of medical oversight in the home. They carry out health assessments of residents, including monitoring vital signs, weight, hydration, and nutrition. They can quickly identify changes in a resident’s health; for example the presence of a new infection, skin that is vulnerable to injury, and changes in the mental state of a resident. The nurses will take action to help residents, in a respectful way.
If needed, Wardington House can provide care for residents with diabetes, cancer, and other long-term health conditions. For example, the nurses can provide catheter care, wound care and PEG feeding. Each resident has a nominated registered nurse, who is responsible for updating care plans and risk assessments, for each of the residents.
Dementia care: The care team at Wardington House has expertise in the care of people with dementia. The nurses and care staff are skilled at understanding and responding to the behavioural changes that arise as a resident’s dementia progresses and if a resident’s health changes. The staff communicate clearly with residents to aid understanding. The nursing team will adjust and develop each resident’s care plans to manage each new situation.
End of life care and palliative care: At Wardington House our registered nurses and nursing assistants play a key role in ensuring comfort and dignity at the end of a resident’s life. Working together with the resident’s GP, the nurses will undertake symptom control and communicate sensitively with families. They will provide a compassionate presence and emotional support to families, in a peaceful setting.
The care team
Matron has a key role in leading the nursing and care team. She communicates with those who are involved in the care of a resident. She ensures that the ethos of the home and standards of care are maintained. Matron ensures that there are nurses on-site 24 hrs per day to support the care team and the residents.
Nursing Assistants: The supervision of teams of nursing assistants is undertaken by the nurses. The nurses allocate duties, conduct handover meetings between shifts, and oversee the delivery of care. The nurses provide clinical guidance and mentorship to care staff. They share clinical issues with Matron and the residents’ GP, when appropriate.
A well managed home
Record keeping and compliance: All care is recorded, including medication, routine care notes, and assessments. These records provide a comprehensive picture of each resident’s health and the records are regularly reviewed. This means that every member of the care team has accurate, up-to-date information about a resident’s needs.
Medication management: Our registered nurses ensure that residents receive the right medication, at the right time, and in the right dose. They check prescriptions, order repeat medications, and liaise with the resident’s GP. They are responsible for administering medication and observing any side effects or adverse reactions, including those which may need attention.
